Short Answer

A balance that resets after restart is usually enforced by a driver app or an audio enhancement. Check the audio control app and disable Enhancements.

Windows stores balance per device, but audio control apps from Realtek or Dell can overwrite it at start-up.

Turning off the enhancements and updating the app stops the reset.

Step By Step Fix
  1. Open Sound settings, Properties, Levels and press Balance, then set L and R to equal
  2. Disable Audio Enhancements in the Advanced tab
  3. Open the Realtek or Dell audio app and reset to default
  4. Update the audio driver and restart

It was the Realtek app. Reset it and fine. I tested it on my own setup and it held up for weeks.

I removed Waves Maxx audio, no more issues. Worth trying before you spend any money.

The Left Right Test showed me instantly which side was weak, so I could stop guessing.
