Short Answer

Windows picks the last connected device. Disable devices you never use, set the default for sound and communications, and check Apps volume and device preferences.

Windows remembers per app output choices as well as the system default, so a headset plug-in can reshuffle both. Disabled and hidden devices still count as options.

Cleaning the list and fixing the defaults stops the sound jumping around.

Step By Step Fix
  1. Open Settings, System, Sound, More sound settings and disable playback devices you never use
  2. Set your main device as both Default Device and Default Communications Device
  3. Open Volume Mixer and check each app output choice, set to Default
  4. Update USB audio and chipset drivers, then restart

Disabling the HDMI outputs I never use was the key for me. I checked it again with the Sound Test afterwards to be sure.
