Short Answer

Bluetooth adds delay, often 150 to 300 ms on basic codecs. Turn on the TV's audio delay or lip sync setting, or use a low-latency transmitter that supports aptX Low Latency.

Standard Bluetooth uses codecs such as SBC that add a noticeable delay while the audio is compressed, sent and decoded. A video player can compensate, but a TV often cannot unless it has a setting for it.

The delay depends on the codec both devices share, so the same headphones can be fine with a phone and late with a TV.

Step By Step Fix
  1. Open the TV sound settings and look for Audio Delay, Lip Sync or Audio Sync Adjustment
  2. Add a small delay to the picture side or reduce the audio delay until the voices match
  3. Update the TV software and the headphone firmware
  4. Use a Bluetooth transmitter plugged into the TV that supports aptX Low Latency, with headphones that support it too
  5. Use wired or 2.4 GHz wireless headphones when watching fast action or sports

My LG TV has a setting called Audio Sync Adjust. Moving it by 100 ms fixed the lips on my Bluetooth headphones.

A cheap aptX Low Latency transmitter fixed it for me. Check that both ends support it.
